Insurances accepted are: Tricare Prime, Select, & Prime Retired; HMSA PPO, Eyemed, and UHA. Exams are $99 with a retinal scan, or $169 with contacts if you opt to see me with an out-of-network plan. (You may be able to get partial reimbursement, check your plan).
If you have not been to the office before, PLEASE read the directions and parking page. Google directions are confusing.
